The percentage of geriatric hospitalized patients requiring post-acute care increased
from 21% in 2000 to 26% in 2015.

More than 80% of these patients required a skilled nursing facility (SNF) stay. SNFs
vary in their ability to provide care, and patients and families are rarely well-enough
informed to choose among their options.

Health decisions that are not based on thoughtful and evidence-based rationale not
only lead to disparities but also add to the cost of health care.
