Factors Associated With Posthospital Nursing Facility Discharge for Patients With Impaired Decision Making

      For some frail patients, a skilled nursing facility (SNF) discharge is the therapeutic bridge needed between the hospital and home to ensure continued independent community living. Little is known about the hospital discharge disposition decision.
