Altarum’s Center for Eldercare Improvement has developed a tool to track progress
within a 3-year comprehensive culture change training and implementation initiative
in 6 Michigan nursing homes. The Systems Change Tracking Tool https://altarum.org/sites/default/files/uploaded-related-files/Altarum-systems-change-tracking-tool.pdf) based on the Holistic Approach to Transformational Change (HATCh) model, has 6 domains:
Care Practices, Workplace Practices, Environment, Family/Community/Activities, Leadership
and Stakeholders. The SCTT captures changes in facility practices over time, providing
data about resident quality of life (QOL) that inform Quality Assurance Performance
Improvement Program (QAPI) Performance Improvement Projects (PIPs).
